We hope to make your visit to our office a positive, informative and pleasurable experience. Please review our policies, and please let us know if there is any other way we can meet your individual needs.

Appointments

Patients are seen by appointment only, except for emergencies. You may call the office during business hours to schedule your appointment. We strive to see our patients as close to their appointment time as possible and know that the patient’s time is as valuable as ours. Emergencies do arise and alter our schedule. In that case, we will contact you and alert you to the change.

Our goal is to provide the best possible care to all of our patients. When we schedule your appointment, a block of time in the doctor’s schedule has been reserved for you, to allow adequate time to provide medical treatment and address any concerns you may have. If you need to cancel or reschedule, please give us at least 24 hours’ notice, so that we may serve other patients at that time.

We reserve the right to charge a $65.00 office fee for all missed appointments and appointments that have not been cancelled at least 24 hours in advance.

Insurance

If you  have been referred by another physician, please inform us at the time of your visit. Some patients, due to restrictions on their health insurance, may need a referral slip from their primary care physician. We are contracted with many HMOs, EPOs, IPAs and PPOs. Please check your provider handbook for more information. The information you provide us with helps us to bill your insurance company without any complications or charges to you, the patient.

Co-payments

If you have a co-pay, please be prepared to pay it at the time of your visit. For your convenience, we accept Visa, MasterCard, cash or personal checks.

Prescription Refills

Our office will provide prescriptions for any medications required as a result of procedures performed by Dr. Wilson. Should you need a refill, please contact our office during business hours, and allow time for the refill to be processed. You may also have your pharmacy contact us to request a refill.

Uninsured Services

Costs for the following items are not covered under insurance. If you are making a request for one of these items, payment must be received at the time of the request.

  • Forms – $10 for the 1st page, $2 for additional pages. Charges can vary depending on the complexity of the note or form required, or time needed to review the medical records for the forms. If you want forms mailed, please provide self-addressed paid envelope.
  • Photocopy Medical Records – $20.00 up to 10 pages, $2.00 for additional pages. 48-hour notification is required for copying medical records. An additional fee can be applied for same-day requests.
  • Fax – $5.00 up to 2 pages, $2.00 for additional pages
  • Returned checks – $25.00 fee
  • Insurance re-billing for incorrect insurance information provided at the time of visit – $10.00 per re-billing.
